So if the little pale blue attachment icon that appears in several different places in several different sizes were a bright RED, would this help you? Maybe this is where possible "skinning" implementation of pmail would be a great thing.
Your other requests depend entirely on the sender's diligence in my opinion. If I'm not mistaken to achieve what you are asking for, ie. name of attachment in header and reminder at end of message body, would require Pegasus to mess with the actual email being sent to you. In my opinion, an mail program should NEVER rewrite received messages, it should only display them if the message's formatting allows.
