Isn't that file name indicative of a temporary file such as is created when one views a received attachment? Is it possible that you sent your template out after having attached a file that you had opened from another e-mail and was not saved discretely? I don't know whether or how that might be possible. I can't imagine that your message was rejected because it had an attachment. Far more likely is that, for some reason, it never went out.
As for preventing attachments to your template-created message, does your template definition specify the inclusion of an attachment? Are you actually using a template or saved stationery?
